Formal Attestation Statement: This is an official agreement specifying that the named participant certifies, and fully acknowledges they are a minority business owner/entrepreneur who is of one (or more) of the following: American Indian/Native American, Alaska Native, Native Hawaiian*, Black/African American, Asian, Asian Pacific Americans, Eskimos, Hasidic Jews, and/or Puerto Rican. Please note: The Census classifies “Native Hawaiian” as Polynesian, Pacific Islander (Pasifika), Samoa, American Samoa, from Micronesia Islands, Tahitian, Tongan, Tokelauan, Marshallese, Palauan, Chuukese, Fijian, Guinean, and Solomon Islander. Additionally, a qualifying business owner/entrepreneur has origins in any of the original peoples of Hawaii, Guam, Samoa, or other Pacific Islands.

Under this agreement, the participant will share information on awarded contracts/jobs, and success stories resulting from receiving direct services.
